Interaction between macrophage and Mycobacterium leprae in the skin lesion : an ultrastructural study

Abstract
Interaction between Mycobacterium leprae and macrophage in the skin lesions of a
lepromatous leprosy case was investigated at the electron microscopic level. Skin
biopsies of a lepromatous leprosy patient, be-fore and after the multi drug treatement,
were extensively studied. In the non-treated skin lesion, more than 50% of bacilli were
look alive. Another half of the bacilli were aggregated with electro leucent material,
which were probably the glycolipid produced by bacilli. The aggregated bacilli did not
look active. Remarkably, most of the bacilli were free in the cytoplasm without any
phagosomal membrane structure around the bacilli aggregate. Some bacilli were even
free at the intercellular space. Dividing bacilli month treatment showed dramatic changes
in their structure within the macrophages. Almost all the bacilli were defined within a
membranous structure which were fused with lysosomes. These bacilli did not look
active. These observations suggested that M. leprae may have ability to evade the
phagosome nd multiply in the cytoplasm. The multi drug treatment may destroy this
ability.
